Clinical Pharmacy Specialist
Summary
Responsible for providing clinical speech services to patients, maintaining and keeping equipment and materials current and assisting with other departmental activities.
Education/Qualifications Required:
- PharmD/Doctor of Pharmacy
- OR Bachelor of Science in Pharmacy AND a PharmD/Doctor of Pharmacy
- AND Hospital Clinical Pharmacy residency of minimum one year duration mandatory
Experience Post Post-Qualifications:
- Minimum two (2) years of experience as a licensed, registered Clinical Pharmacist, (after Residency) in the specialty for which applying (i.e., oncology or critical care), in a tertiary hospital setting
- Arabic speaker is an asset
License:
- Current license as a Registered Pharmacist
Job Duties and Responsibilities:
- The main responsibilities of the job holder are to provide clinical pharmacy services, including advising health care providers on matters pertaining to drug selection while assessing for optimal drug therapy, dosage regimen, effective administration and drug therapy monitoring; and participate in the development and delivery of continuing education programs for pharmacy, patient care services, medical staff and students. They may also be providing non-clinical pharmacy services in another part of the pharmacy service [Patient/Technical or Quality Assurance (P/T/QA) service].
1. Patient Care:
- Conduct medication reconciliation; identifying the most complete and accurate list of medications a patient is taking and using this list to provide correct medications for the patient anywhere within the organization upon admission or transition of care.
- Participate in the multidisciplinary ward rounds/meeting
- Assess current medication therapy; patient’s medical record should be reviewed in conjunction with the medication administration record and medication history obtained.
- Recent consultations, pathology results and investigations, treatment plan and daily progress should be taken into account when determining the appropriateness of current medication therapy and when planning patient care
- Prevent, detect, and resolve medication-related problems, make appropriate evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary team.
- Evaluate response to drug therapy using patient’s existing parameters.
- Therapeutic Drug Monitoring (TDM): interpretation, monitoring, and communication of measured drug concentrations in body fluids to optimize medication efficacy and minimize toxicity.
- Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
- Provide medication counseling throughout patient’s journey.
- Management of medication errors and adverse drug reactions.
2. Conduct and participate in Drug Use Evaluation (DUE) to evaluate and improve medication use process
3. Participate in development of clinical guidelines, protocols or pathways
4. Research:
- Perform and collaborate on research project
- Disseminate research results through meetings, conferences, or publications
5. Complete the Continuing Professional Development (CPD) requirements (e.g. credit hours) for maintaining a pharmacist license in the state of Qatar or the HMC requirements if these are higher
6. Training and Education:
- Provide educational activities for pharmacy staff and other healthcare providers e.g. physician and nurses
- Participate in the orientation and training of the new staff and pharmacy students
- Participate in patient education, e.g. presentations and education programs
- Development of patient information materials
7. Ensure appropriate and safe system of care in collaboration with other healthcare professionals (Nurses, physicians, etc…)
8. Documentation:
- Write in the medical record to communicate with clinicians and to document pharmacist contributions to patient care as per HMC policies and procedures
- Documentation of clinical pharmacist interventions
- Report medication errors and adverse drug reactions
9. Medicine Distribution Responsibility:
- Provide operational support in the pharmacy as determined by departmental needs
- Provides leadership and oversight for pharmacy technician.
- Maintains knowledge of changes in medication delivery systems to provide accurate and timely service.
- Demonstrate understanding of the process to access medications through the various technological systems.
- Participate in opportunities to improve the quality, effectiveness, and/or efficiency in distributing medications to patients.
- Demonstrates quality and safety in the distribution process.
- Actively collaborates and demonstrates teamwork with pharmacy teams to ensure drug distribution is communicated and delivered safely and effectively.
